Be sure to SIGN UP by clicking on the option below before you schedule your first session/class. This will give you the oversight of both scheduling, canceling and rescheduling your session/class seamlessly. You can also track your available balance of classes/sessions based on the package you have purchased with the unique code provided to you at time of purchase.
LIABILITY WAIVER, CANCELATION POLICY & EMERGENCY CONTACT FORM
All clients must complete a Liability Waiver, Cancelation Policy and Emergency Contact form prior to attending their first session/class. Appointments can only be canceled/rescheduled 24 hours in advance of scheduled time. Appointments can not be scheduled on the same day as service without speaking to Theresa first.
24 hours notice prior to your scheduled appointment must be given to cancel your training appointment or your package will be charged a session. You can work with me to rescheduled appointments with less than 24 hours notice but it must be rescheduled for the same day to save that session; based on availability. If you cancel within 24 hour time period or cannot reschedule the same day and I can get another client to fill your time slot, your package will not be charged. This policy not only gives you an incentive not to miss your session, but also protects the trainer as the facility must be paid by the trainer for “no-shows”.

APPOINTMENTS FILL UP FAST!!!
Due to demand, clients may not be able to book repeat appointments for the same day/time each week. Clients are encouraged to book appoints as far in advance as possible to maximize the chance of securing a regular weekly schedule.
